| Mandatory Environment | Do not run this session in a community setting unless a VA‑or‑contract clinician (or 988 responder briefed on roster) is reachable by phone or secure video within 5 minutes with written/chat agreement. On‑site: Designated “Safety Zone” staffed; 988 Press 1 and local VA emergency contacts physically visible; unlocked door, visible exit. |

Capitol Contracts LLC assumes no liability for acute psychiatric management beyond documented SPI handoff compliance.”
SECTION 2: PRE‑SESSION FACILITATOR CHECKLIST (Do not proceed unless all boxes checked)
- SPI/warm handoff pathway confirmed and tested.
- Local crisis resources printed and visible (988 Press 1, 741741, facility lines).
- Appendix B (Local Crisis Pathway) fully filled out and each number test‑called within 24 hours.
- Safety Reference Cards (full template – see Section 10) pre‑printed (1 per participant + 5 extras).
- C‑SSRS Brief Screener (Section 23) printed (1 per participant).
- CSRE escalation pathway confirmed with on‑call clinician (verbal or written agreement).
- Clinical supervisor identified for SBOR filing (name and contact added to Appendix B).
- Safety Zone identified and staffed/on‑call.
- Facilitator self‑regulation completed.
- S‑12‑SAF fillable PDF printed or downloaded (see Section 15).
- Participants screened: prior sessions completed; no active ideation with intent/plan (if present, individual handoff – do not proceed with group).
- Means safety materials present: minimum 5 gun locks + 5 medication lockboxes. If not, reschedule.
- Means Safety Acknowledgment Log (see Section 11) printed.
- For low‑resource delivery: confirm all Appendix C pre‑conditions.
- Post‑session debrief time allocated (2 minutes per participant).
- Facilitator Agreement & Signature Page (see Section 17) signed and filed.
